Your Guide to Completing the DH Ultimate Order Form

We’ve put together this simple step-by-step guide for your reference to make filling out your DH Ultimate order form easy and hassle-free.

We want to make the ordering process as smooth as possible, especially when ordering multiple designs, sizes, and player details. Just follow the steps below, and you’ll be able to complete your order form quickly and accurately.

Step 1: Complete Your Order Details

Start by filling out the order details, shipping information, and desired deadline. Make sure all information is complete and accurate so DH Ultimate can process the order smoothly.

Providing the deadline also helps DH Ultimate plan the production and shipping schedule to keep everything on track.

Step 2: Fill Out the Order Sheets

There are three available sheets for jersey orders: Tops 1, Tops 2, and Tops 3. These sheets are used to organize orders with multiple jersey designs.

For shorts and pants, you may use the Bottoms 1 and Bottoms 2 sheets if you have two different designs.

For headband and towel orders, you may enter them in the Accessories sheet, while cap orders should be entered in the Hats sheet.

For each design, fill out the corresponding sheet with the required order details, including the jersey color, fabric, and any additional instructions. 

Please note: The name to be printed column is case-sensitive. DH Ultimate will print the name exactly as it is entered in the order form, including the use of uppercase and lowercase letters. Please double-check the spelling and capitalization before submitting the order form.

For example:

• Entering JORDAN will result in JORDAN being printed.
• Entering Jordan will result in Jordan being printed.

For reversible apparel, you only need to list the item once on the appropriate design sheet. There is no need to duplicate it on both the light and dark sheets.

For orders of 7 pieces or more of reversible jerseys, you may use a separate sheet specifically for the reversible jerseys to keep your order details organized.

Step 3: Special Requests 

If you have any special requests, please indicate them clearly in the Special Request column. This may include fit adjustments or custom measurements.

Additional 2 inches sleeve length.

Step 4: Double-check Player Details 

Please review all player details carefully before submitting your order form. Make sure each player's name, shirt type, printed name, jersey number, and size are correct.

Step 5: Order Form Submission 

Once everything is confirmed and complete, please email the finalized order form to DH Ultimate for review.

Step 6: What Happens Next? 

Once we receive your completed order form, our team will review the details and guide you through the next steps.

1. Order review: We’ll check your order details and follow up if any information needs clarification.

2. Down payment: Once the order details and designs are ready, we’ll send over the invoice to you for your reference.

3. Production: Your order will proceed to production once we have received the order form and down payment and finalized the designs.

4. Order updates and shipping: We’ll keep you updated on the progress of your order and share the tracking details once your order has been shipped.
